Your moisturizer is supposed to be the safest product in your routine. And yet, for millions of people with reactive or sensitive skin, it's often the source of redness, stinging, or breakouts. Why? Because most moisturizers - even those marketed as "gentle" or "for sensitive skin" - contain ingredients that are well-known irritants.
Here are the 12 most common offenders, what they do, and why they can be a problem.
1. Fragrance (Parfum)
Fragrance is the single most common cause of contact dermatitis from cosmetics. The term "fragrance" or "parfum" on an INCI list can hide a blend of dozens of individual chemicals, many of which are potent allergens. Even products labeled "unscented" may contain masking fragrances. If you have sensitive skin, fragrance-free formulations are your safest bet.
2. Methylisothiazolinone (MI)
MI is a preservative that became widely used in the 2000s as an alternative to parabens. It is now one of the most frequently flagged allergens in Europe, and many countries have restricted or banned it in leave-on products. It can cause both irritant and allergic contact dermatitis, even at low concentrations.
3. Methylchloroisothiazolinone (MCI)
Often used in combination with MI, MCI is even more potent as a sensitizer. The combination MCI/MI (sold under trade names like Kathon CG) is permitted only in rinse-off products in the EU at limited concentrations. If you see it in a moisturizer, that's a red flag.
4. Propylene Glycol
A humectant and solvent used in countless formulations. Most people tolerate it well, but at higher concentrations (above 5%), it can cause irritation and stinging in sensitive individuals. It is also a penetration enhancer, which means it helps other ingredients (including potential irritants) absorb deeper into the skin.
5. Sodium Lauryl Sulfate (SLS)
Primarily a surfactant used in cleansers, SLS occasionally appears in creams and lotions as an emulsifier. It disrupts the skin's lipid barrier and increases transepidermal water loss (TEWL). Even at low concentrations, regular exposure can degrade barrier function over time - the opposite of what a moisturizer should do.
6. Alcohol Denat. (Denatured Alcohol)
Alcohol denat. evaporates quickly, giving products a light, non-greasy feel. But it strips the skin's natural lipids and disrupts the barrier. With repeated use, it can cause chronic dryness and sensitization. It's particularly common in "oil-free" or "mattifying" moisturizers. Look for it high up on the ingredient list - the higher it appears, the more of it there is.
7. Essential Oils
Lavender, tea tree, eucalyptus, citrus - these are all popular "natural" ingredients with well-documented sensitizing potential. Essential oils contain dozens of volatile organic compounds, many of which are classified as allergens under EU cosmetics regulation. "Natural" does not mean non-irritating.
8. Formaldehyde-Releasing Preservatives
DMDM hydantoin, imidazolidinyl urea, diazolidinyl urea, and quaternium-15 all slowly release small amounts of formaldehyde to prevent bacterial growth. Formaldehyde is a known human carcinogen and sensitizer. Low-level, repeated exposure is associated with contact dermatitis.
9. Lanolin
Lanolin is an excellent emollient derived from sheep's wool. It is highly effective for dry skin, but it is also a common allergen - particularly for people with eczema or psoriasis, who are already more prone to sensitization. If you've had reactions to wool clothing, lanolin is worth avoiding.
10. Cocamidopropyl Betaine
A mild surfactant derived from coconut oil. While gentler than SLS, it is a recognized allergen and is one of the more common causes of contact dermatitis in personal care products. The impurities formed during its manufacturing process are the primary culprits.
11. Parabens (in sensitized individuals)
Parabens have a generally good safety record and are considered low-risk for most people. However, individuals who have already developed contact dermatitis or compromised skin barriers can become sensitized to them. Methylparaben, ethylparaben, propylparaben, and butylparaben are the most common variants.
12. Citric Acid (at high concentrations)
Citric acid is used as a pH adjuster and mild exfoliant. At low levels it is benign. At higher concentrations, it can cause stinging and irritation, particularly around the eyes and on skin with a disrupted barrier. It's not inherently dangerous, but it's worth watching in products you apply to sensitive areas.
A note on "sensitivity" vs. "allergy": Irritant contact dermatitis (ICD) and allergic contact dermatitis (ACD) are different reactions. ICD happens when an ingredient directly damages the skin - anyone can get it with enough exposure. ACD is an immune response that only occurs after sensitization. Both look similar but are caused by different mechanisms.
